TITLE:  Care Manager/Health Coach (2 positions available)

REPORTS TO:  Michael Skinner, LCSW, Program Director, Children’s Health Homes and SI Cares

QUALIFICATIONS (Educational/Knowledge Requirements): 

1.  Relevant experience in care management and child/adolescent population
2. Ability to interact with physicians and other health care professionals in a professional, responsible manner

3. BS/BA with relevant health care experience
4. Spanish-speaking preferred 

RESPONSIBILITIES (include, but are not limited to the following):

· Serves as a link between Primary Care Physician (PCP), educational staff and client’s mental health provider to improve coordination of services and information flow;
· Interacts with PCPs and educators in order to inform professionals of behavioral health disorders and treatment options available for the clients they serve;
· Provides ongoing contact with clients;
· Provides coordination and linkage to general medical/educational services;
· Works with client’s providers to customize services to best meet client’s needs within the parameter of available services;
· Meets with client’s primary care, behavioral health providers and educational staff, and other staff as needed to develop and coordinate care/treatment plans and meets with client’s family members as needed; 
· Interfaces with and refers clients to other supportive services as appropriate;
· Adheres to policies and practices as stipulated in the agency’s Policy and Procedures Manual; 
– Prepares timely documentation of all client activity as required by contractual obligations; 
· Attends In-Service Training Sessions throughout the year;  
· Other duties as assigned by management.
